Medical Records

The Medical Records department is responsible for coding, managing requests, and releasing medical information.

Due to the confidential nature of a patient’s medical record, Hudson Regional Hospital requires all requests for the release of medical records to be accompanied by a completed authorization form signed by the patient. Patients must complete a “Release of Information” form and the Medical Records Staff will provide a complimentary copy of the record. For multiple long stays, the Medical Records Staff may need to review and copy the medical records and call the patient when they are completed and ready for pick up.

In Person Medical Records Requests

You can also submit your request in person by completing the “Authorization for Release of Information” form. You can obtain the form in the hospital’s main lobby or the Medical Records office on the 2nd floor, or download it below. ID is required when requesting or picking up records in person. If you are picking up another person’s records, you will need additional legal documentation or written authorization.

There is no fee if the record is being released to a doctor or healthcare provider for continuity of care. However, there are reproduction and postage/handling fees as applicable, for other types of requests.
